Ukraine helicopter crash kills interior minister, others

On 18 January 2023, around 06:10 UTC during flight en route Kozyn – Novi Petrivtsi – Poltava, with the officials on board, by helicopter EC 225 LP, reg. 54 (STATE AVIATION of Ukraine), operator “State Emergency Service of Ukraine”, during flight above Brovary city, rotorcraft collided with the building of the kindergarten “Dzhereltse”. As a result helicopter was totally destroyed. The 10 crew and passengers on board were killed. 4 persons, that were on the ground, sustain fatal injuries, 25 person sustain injures different level of severity.
Eyewitness testimony described the weather conditions in the area as foggy, with witnesses reportedly seeing the helicopter on fire, and circling towards the ground with its tail boom and rotor missing, before crashing into the ground.
Twenty-five people located in and around the kindergarten were taken to Brovary City Hospital, of which 8 had suffered serious burn injuries. Due to the deteriorating situation in Ukraine following the ongoing conflict, 6 of these patients were sent outside the country for further specialized treatment.

During flight above Brovary city, rotorcraft collided with the building of the kindergarten “Dzhereltse” and burst into flames.High profile passengers/crew members:
* Commander Oleksandr Vasylenko
* First Officer Kostiantyn Kovalenko
* Flight Engineer Ivan Kasianov
* Minister of Internal Affairs Denys Anatolievych Monastyrskyi
* Deputy Minister of Internal Affairs Yevhen Volodymrovych Yenin
* Secretary of State for the Ministry of Internal Affairs Yurii Olehovych Lubkovych

In addition, the chief of security for Monastyrsky, as well as 3 staffers for the Ministry of Internal Affairs were killed in the crash.

The helicopter was one of 21 H225 aircraft delivered to Ukraine as assistance in replacing the countries aging fleet of Soviet aircraft, such as the Mil Mi-8. The helicopter was originally delivered to a customer in Norway in 2009, and was acquired by the State Emergency Service of Ukraine after being refurbished in 2020 following periods of ownership in Norway, Brazil and France.

On 3 August, the State Bureau of Investigation of Ukraine announced that five State Emergency Service employees had been charged with safety violations and negligence regarding the crash, namely the head of the service's Department of Aviation and Air Search and Rescue, its acting commander, the commander of the aviation squadron, the deputy commander for flight training, and the head of the flight safety service of the Special aviation unit from Nizhyn, Chernihiv Oblast.
The investigation established that the officials had committed blatant violations of air traffic safety and operational rules, which lead to the deaths of people.

State Bureau of Investigation findings:
It was established that in January 2023, a visit of the top officials of the Ministry of Internal Affairs of Ukraine from Kyiv to the Kharkov and Dnepropetrovsk Oblasts was planned.
Despite the internal instructions and legal requirements, State Emergency Service officials used a helicopter that was designated to provide emergency air services for the city of Kyiv and the surrounding area. The aircraft had no permits or verification for any other types of flights. During the preparation for the flight, the crew commander was not informed of the meteorological data along the route of the planned flight. Of particular interest there were unfavorable weather conditions over the city of Brovary. In addition the crew did not have the appropriate flight plans, or certifications to fly in difficult weather conditions. However, none of the officials responsible for flight safety cancelled the flight, or at least postponed it until favorable weather conditions improved.
Due to difficult weather conditions, the helicopter was forced to fly at an extremely low altitude, even lower than the height of high rise buildings along the route. From this, the cause of the crash is attributed to the pilot attempting to maneuver over an oncoming building, but lost spatial orientation in the poor weather and crashed into the ground.

When the helicopter crashed, it damaged a kindergarten, neighboring buildings and parked cars.

Officials of the Aviation Department of the State Emergency Service of Ukraine were served with a notice of suspicion of violating flight safety rules (air traffic safety), which caused the death of people and large scale material damage, i.e. of committing a criminal office under Article 276 of the Criminal Court of Ukraine.

The head of the flight safety service of the same unit was served a notice of suspicion under Article 367 of the Criminal Code of Ukraine (official negligence).

On 29 August 2024, Ukrainska Pravda reported that the case had been transferred to court. Prosecutors filed an indictment against five officials of the State Service of Ukraine for involvement in the disaster. The investigation concluded that the accused grossly violated flight security rules which carried on board the leadership of the Ministry of Internal Affairs. Foreign experts were involved in the investigation, including representatives from the helicopters manufacturer Airbus Helicopters (formerly Eurocopter).

В МВД Украины назвали возможные причины крушения вертолета в Киевской области 18 января 2023 года. По данным ведомства, рассматривается три основные версии: диверсия, неисправность техники, нарушение правил безопасности. В районе катастрофы был густой туман.
Там также уточнили, что в результате падения вертолета погибли министр внутренних дел Украины Денис Монастырский и его заместитель Евгений Енин.
Напомним, утром 18 января в городе Бровары в Киевской области упал украинский вертолет EC 225 LP (54). Он рухнул прямо рядом со зданием детского сада. Во время инцидента в саду находились дети и сотрудники учреждения. После крушения начался пожар. В результате случившегося погибли 10 человек на борту вертолета и 4 на земле, пострадали - не менее 25. Среди жертв есть дети.

На 18 януари 2023, около 06:10 UTC при полет по маршрут Козин - Нови Петривци - Полтава, вертолет на украинската служба за извънредни  ситуации EC 225 LP (54)  се удря в сградата на детската градина "Джерелтце" в гр. Бровари. В резултат вертолетът е напълно разрушен, а мястото на катастрофата е обхванато от пламъци. Загиват 14 души, от които 10 на борда и 4-ма на земята. Най-малко 25 души получават различни по тежест наранявания. Сред загиналите на борда са лица от ръководството на МВР на Украйна, в това число министърът Денис Монастирски и неговият заместник Евгений Енин, както и 3-членният екипаж. Полетът се провел в условия на гъста мъгла, за което екипажът не бил подготвен.

При разследването са разглеждани три версии за произшествието: диверсия, техническа неизправност и нарушаване правилата за безопасност. Предполага се, че екипажът е изпаднал в състояние на пространствена неосведоменост поради мъглата, което довело и до катастрофата.
