2 Israel pilots killed in navy helicopter crash

An IDF Eurocopter AS 565MA Atalef (reg. 895), operated by 193 Squadron, crashed into the sea at night off Bat Galim beach, Haifa. The helicopter had been airborne for about an hour, taking part in a training exercise at the time of the accident.
The second interim report about the deadly helicopter crash off the coast of Haifa has determined that the cause was a broken blade in the left engine that caused a fire that also damaged the right engine and caused the helicopter to crash.
Two IDF pilots, Lt.-Col Erez Sachaini and Maj. Chen Fogel, died after their helicopter- an Atalef AS565 Panther − went down after a fire broke out in the left engine and crashed off the coast. The third crew member, naval officer Cpt. Ron Birman survived.

Two dead in army helicopter crash in Tunisia

A Tunisian military helicopter Agusta-Bell AB 205A crashed in the north of the country, killing the pilot and the co-pilot. The helicopter was conducting a reconnaissance mission in Bizerte governorate. An eyewitness reported seeing the aircraft land in the field, takeoff and fall from a height of 100 m.

The single-engine, five-seat Piper Cherokee Six plane (N5535J) was heading to Chame from Contadora island, about 50 miles (80 kilometers) away, but splashed into the Pacific and sank. The crash occurred off Panama’s Punta Chame, south of Panama City. Of the 5 persons onboard, the pilot and two passengers received serious injuries, and 2 passengers were fatally injured (Debra Velleman, a retired Waukesha school teacher and Sue Borries, 57, of Illinois). Their bodies were found nearly a month later. Before the accident the pilot had radioed to say he was having engine problems and would have to splash down in the sea. The pilot later told authorities the aircraft sank so quickly they were not able to grab a life raft or life jackets.

Final report

On Monday morning, 3 January 2022, a Jetstream 41 aircraft with registration ZS-NRJ was on a scheduled commercial flight from O.R. Tambo International Airport (FAOR) in Gauteng province, to Venetia Mine Airfield (FAVM) in Limpopo province. On-board the aircraft were three crew members (two pilots and one cabin crew) and four passengers. A flight plan was filed for this flight and the flight was conducted under instrument flight rules (IFR) by day and under the provisions of Part 121 of the Civil Aviation Regulations (CAR) 2011 as amended.
A safety briefing was conducted by the cabin crew member before departure. After take-off, the aircraft climbed to flight level 210 (FL210). Approximately 15 nautical miles (nm) from FAVM and during descent, the first officer (FO) made a radio call to the FAVM attendant on the very high frequency (VHF) 124.80-Megahertz (MHz) to communicate that they will be landing shortly. Thereafter, the airfield attendant performed a runway inspection and, after completion, informed the crew that the runway was clear. The aerodrome attendant saw the aircraft whilst on final approach and after it touched down on Runway 08. During the landing roll at a ground speed of approximately 43 knots (kt), a large bird (Kori Bustard) got airborne from the right-side of the runway with overgrown grass and flew towards the aircraft. Soon after, one of the right-side engine propeller blades hit the bird.
The aircraft was substantially damaged, however, the occupants on-board the aircraft were unharmed.

Probable Cause
A bird strike on the propeller blade caused the overload fracture and separation failure of the blade.
Contributory Factors:
1. Inadequate control of wildlife at the airfield.
2. Inadequate number of trained personnel to monitor and deter possible wildlife during aircraft operations.

7 dead in tourist plane crash near Peru's Nazca lines

Final report

Five tourists and two crew died when their plane crashed on a trip to view Peru's famous Nazca lines. The Cessna T207A Turbo Stationair 8 single-engine plane (OB-2179) belonging to the Aero Santos tourism company came down shortly after takeoff from the small airport of Maria Reiche in Nazca around noon. There were no survivors among the seven on board. The tourists were two Chileans and three people from the Netherlands

PROBABLE CAUSE
Progressive loss of engine power due to malfunction of the combustion system from the start of the takeoff, which significantly compromised the performance of the aircraft in flight, as it could not maintain adequate speed and altitude to return to the airport, resulting in a sudden loss of aerodynamic lift and subsequent impact with the ground.

CONTRIBUTING FACTORS
1. Deficiencies in Maintenance and Supervision by OMA No. 055 and the Aircraft Operator:
a. Poor periodic maintenance of the engine's spark plugs and cylinders.
b. Installation of a "HOT" range thermal classification spark plug, not compatible with the engine, which worsened its performance and affected the aircraft's performance.
c. Failures in the maintenance, supervision, and certification procedures by the OMA.
d. Failures in the technical supervision by the Aircraft Operator, specifically in the processes of reception and quality of the work performed on the aircraft by the OMA.

2. Deficiencies in Crew Training and Education:
a. Using an incorrect configuration (0° flaps instead of 10°) for takeoff with weights very close to the allowed MTOW (Maximum Takeoff Weight).
b. Lack of proper operational action during takeoff by not following the emergency procedures established in the aircraft's CHECKLIST and POH (Pilot's Operating Handbook).

A Pilatus PC-12/47E, N79NX, headed from Engelhard, NC (7W6) to Beaufort-Michael J. Smith Field, NC (KMRH) when it impacted the water of the North Atlantic Ocean off Beaufort, North Carolina, USA. Eight people, including four East Carteret High School students, died in the crash as the group was returning from a duck hunting trip in Hyde County. Federal investigators point to pilot error as the cause of a plane crash.
The NTSB says pilot Ernest “Teen” Rawls, of Greenville, didn’t adequately conduct preflight planning, had inadequate inflight monitoring of the airplane’s flight parameters, and failed to regain control of the airplane following entry into an inadvertent aerodynamic stall brought about the accident.
The NTSB says the pilot’s likely disorientation in the cockpit following the aerodynamic stall also contributed to the crash.
The pilot's medical conditions of mantle cell lymphoma, back pain, and recent history of COVID-19 and the medications used to treat these conditions, including rituximab, oxycodone, hydroxychloroquine, and ivermectin, were contributing factors to this accident.

A Cessna 208B Grand Caravan EX plane, with registration 5H-MZA, crashed into the sea in Comoros with 14 people on board. The aircraft, owned by FLY ZANZIBAR LIMITED and chartered by AB AVIATION of Comoros, was on flight Y61103 between Moroni and Mohéli when it crashed while attempting to land at Mohéli-Bandar es Eslam Airport (NWA), Fomboni, Comoros – on Mohéli Island. AB Aviation announced the disappearance of the plane that took off from Moroni at 5:50 a.m. and disappeared from radar, over the sea, 2.5 km from Mohéli airport. The pilots were citizens of Tanzania, while the passengers were all Comorians. After 24 hours of intense research, only few debris were found floating on water (such a wheel and wing fragments). No trace of the 14 occupants was found. Reports indicate that there have been recent bad weather conditions at sea in the Comoros Islands.
A body was found in the sea near the crash site in March 2023. The body has been taken to Moroni for further examination. It is believed to be one of the passengers on board the crashed aircraft.

Seven killed in helicopter crash in Italy's Monte Cusna

An Agusta A119 Koala (I-ELOP), that took off from Lucca Airfield (LIQL) towards a paper company in Castelminio di Resana (near Treviso Airport TSF/LIPH) with 7 (1 Italian pilot, 4 Turkish and 2 Lebanese citizens) people on board, disappeared from radar screens Thursday morning as it flew over the province of Modena in the Tuscan--Emilian Apennines. Electric storms had been reported in the area at the time.

The two Lebanese were identified in Lebanon as Shadi Kreidi and Tarek Tayah, both executives at INDEVCO, an international manufacturing and industrial consultancy group. The two were said to be on a business trip. Tayah's wife, Hala, was killed two years ago in the massive explosion at Beirut's port, which took the lives of more than 215 people and injured thousands. The seven deceased occupants were found Saturday 11-th June. The helicopter was found on Mount Cusna after being reported by a hiker. The area of the crash is a ravine on Mount Cusna, in the area of Villa Minozzo (Reggio Emilia), at an altitude of about 1,900 meters.

A Mexican Navy Sikorsky UH-60M Black Hawk helicopter (ANX-2307) crashed in Mexico leaving 14 occupants dead following the arrest of drug lord Rafael Caro Quintero. Another passenger was receiving medical treatment after the accident in the north-western state of Sinaloa.

They had all supported the arrest operation. Caro Quintero, the co-founder of the former powerful Guadalajara cartel, was captured on Friday in San Simón, 135 kilometers (83.89 miles) to the north, near Choix.
Mexican President Andrés Obrador regretted the loss and expressed his condolences to the families of the deceased on social media.
The crash had occurred afterward while landing in the locality of Los Mochis, and the causes were being investigated. The investigation file claims, that there was no external attack, nor any explosion caused by any device other than the aircraft. Regarding the black box, the manufacturer, after receiving and analyzing it, has indicated that a lack of fuel was the cause of the crash.

Twenty-two soldiers were killed when a Ugandan military helicopter Mi-171E (AF-641) , crashed Monday in the Boga region of the Ituri province, in northeastern Democratic Republic of the Congo (DRC).
The dead included three Congolese soldiers, said the source, who spoke under customary anonymity, without offering further details.
On Tuesday, another helicopter belonging to the Uganda army crashed at the Kabarole border area between the two countries, during joint operations between the two armies to hunt down the Allied Democratic Forces (ADF), a rebel group. According to the Congolese army's media service, the helicopter hit a cluster of power lines. A Ugandan soldier and a Congolese soldier died in the crash that happened Tuesday.

Air tanker crash in Italy kills two

A Canadair CL-215/415 ("Canadair 28") crashed while dropping water on a wildfire on the slopes of the volcano Etna near Linguaglossa, Sicily, Italy.

Both pilots, Matteo Pozzoli and Roberto Mazzone, were killed.
The air tanker had just released its load, banked sharply to the right, then the Canadair’s right wing impacts the down slope of a mountain, causing the rest of the airframe to collapse, hit the ground and explode.
The registration number of the aircraft was I-DPCN. It was part of a fleet of 19 CL-415s owned by the Dipartimento dei Vigili del Fuoco, del Soccorso Pubblico e della Difesa Civile (Department of firefighters, public rescue and civil defense) but operated by Babcock, the company that provides the Canadair service in Italy as part of an outsourcing contract.
As a side note, Canadair 28 was the second Italian CL-415 to be given the civilian registration I-DPCN. The first one crashed near Brescia on Aug. 16, 2003. At the time, that airframe I-DPCN was coded 9 (“Canadair 9”). The aircraft was damaged beyond repair, while the two pilots survived the incident with wounds (one of them was heavily wounded).
