B-52 crashes in Guam

A B-52 Stratofortress bomber (60-0047) crashed on takeoff Thursday morning at a U.S. Air Force base in Guam during a training mission, with all seven crew members safely leaving the aircraft, the base said. One of them received minor injuries. The huge bomber experienced a runway excursion during a high-speed, heavy-weight aborted takeoff at Andersen Air Force Base about 8:30 a.m. (6:30 p.m. Wednesday ET).
The B-52's home is Minot Air Force Base in North Dakota. The crew members are from the 69th Expeditionary Bomb Squadron.
The Accident Investigation Board found that the cause of the mishap was that the pilot analyzed visual bird activity and perceived cockpit indications as a loss of symmetric thrust required to safely attain flight, and subsequently applied abort procedures. Drag chute failure on deployment and exceeding brake-energy limits also substantially contributed to the mishap.
Military officials have said Guam provides a much different training environment than North America, part of which is the ability to take part in joint exercises.
B-52s cost about $84 million apiece, according to the military.
Guam is a U.S. territory in the Pacific, about 3,800 miles west of Honolulu and 2,500 miles southeast of Beijing.

Самолетът бил предислоциран от авиобазата в Минот, Северна Дакота, а екипажът е от 69-а транспортно-бомбардировъчна ескадрила.

Стойността на един такъв самолет е около 84 млн $, според американски военни.

 List of the crew of the An-12 (4K-AZ25)

 

A Silk Way Airlines cargo An-12 plane (4K-AZ25) carrying nine people has crashed after taking off from Dwyer airport in Helmand province in southern Afghanistan. Seven of  9 people on board were killed. Two men survived the crash and their health condition is stable.The accident was happened during the departure from the airport Dwyer. The AN-12 was leased from Azerbaijan's Silk Way for cargo transportations in Afghanistan. The aircraft landed in the Dwyer airport after leaving Afghanistan's Bagram airfield and was flying to Mary, Turkmenistan, for refueling. There was no cargo on board.

The State Civil Aviation Administration of Azerbaijan noted that the accident happened at about 06:30 p.m. Baku time on Wednesday. Nine people were on board the aircraft, including five citizens of Azerbaijan, three citizens of Ukraine, and a pilot from Uzbekistan. Azerbaijan State Civil Aviation Administration established a commission to investigate the accident.

The debris of the crashed rescue aircraft CASA C-212 Aviocar 400MPA (Video)

Rescue forces found several bodies identified as crewmembers of the crashed rescue aircraft CASA C-212 Aviocar 400MPA as of 4pm of June 23.
They determined the position of the aircraft, registered 8983, at a depth of between 50-60 metres under the sea, 15 nautical miles south-southeast of Bach Long Vi island of northern Hai Phong city.
Several pieces of debris, including propeller and fuselage with numbers 8983, were found around the scene.
On June 16, the CASA 212 with nine crewmembers on board was dispatched after rescue units received a help signal believed to be from Senior Lieutenant Colonel Tran Quang Khai, who was the missing pilot aboard fighter jet SU30-MK2 that went down during a training session two days earlier.
After passing Bach Long Vy island district in the Tonkin Gulf by 44 nautical miles, the plane asked for permission to descend but contact was then cut. The plane crashed.
All 9 occupants onboard died.
On the afternoon of June 23, search forces for the SU30-MK2 discovered aircraft debris at latitudes between 18 degrees 57 minutes to 19 degrees North and longitudes from 106 degrees 3 minutes to 106 degrees 4 minutes East on the waters of central Nghe An province. The position was first determined to be the place where the aircraft crashed.
The SU30 MK2 aircraft with two pilots went missing at 6:50 am of June 14 in the area of Hon Mat Island, about 40km off the coast of the central province of Nghe An’s Vinh city. One pilot was rescued by fishing boat and brought ashore safely. “There was an explosion in the cockpit. I had to make a parachute jump,” the rescued pilot, Nguyen Huu Cuong, told state media after he was brought to shore.

The body of Khai was found on June 17 and brought ashore on June 18.

The four-engine military transport airplane, a Lockheed C-130 Hercules (16804) caught fire during take-off at the Montijo military base in Portugal on Monday.

Three transport squadrons and one helicopter search and rescue squadron are located at the Montijo Airbase. The crashed aircraft was part of 501st Squadron known as the Bisons, providing logistic support to the Portuguese Navy.

The crew was involved in a local training flight in Montijo AFB, consisting of touch and go maneuvers. During the takeoff roll on runway 26, the four engine aircraft deviated from the centerline to the left and then went out of control, veered off runway to the right and  crashed in flames 1,460 meters past the threshold.

There were seven people on board the cargo plane, all of them military, with three fatal victims, at least one more person was seriously injured and 3 others escaped with minor injuries. The aircraft was partially destroyed by a post crash fire.

The incident happened around noon local time. Dozens of emergency teams have been mobilized following the crash. The turboprop C-130 Hercules model has been produced for over 60 years, making it one of the longest continuously-manufactured military aircraft. The cargo plane serves as the main tactical airlifter for many military forces in the US, Europe and other parts of the world.

Preliminary report of the investigation

Emirates Flight 521

 

Emirates Airlines flight EK521, a Boeing 777-300 (reg. A6-EMW), impacted the runway during an attempted go-around at Dubai Airport (DXB), United Arab Emirates. All 300 on board survived the accident. One fire fighter was killed during the ARFF operations.The aircraft departed Thiruvananthapuram, India at 05:06 hours UTC. The captain was the pilot flying (PF), and the copilot was the pilot monitoring (PM).

As the flight neared Dubai, the crew received the automatic terminal information service (ATIS) Information Zulu, which included a windshear warning for all runways. The aircraft was configured for landing with the flaps set to 30, and approach speed selected of 152 knots (VREF + 5) indicated airspeed (IAS) The aircraft was vectored for an area navigation (RNAV/GNSS) approach to runway 12L. Air traffic control cleared the flight to land, with the wind reported to be from 340 degrees at 11 knots, and to vacate the runway via taxiway Mike 9.
During the approach, at 08:36:00 UTC (12:36 local time), with the autothrottle system in SPEED mode, as the aircraft descended through a radio altitude (RA) of 1,100 feet, at 152 knots IAS, the wind direction started to change from a headwind component of 8 knots to a tailwind component.
The autopilot was disengaged at approximately 920 feet RA and the approach continued with the autothrottle connected. As the aircraft descended through 700 feet RA at 08:36:22, and at 154 knots IAS, it was subjected to a tailwind component which gradually increased to a maximum of 16 knots.
At 08:37:07, 159 knots IAS, 35 feet RA, the PF started to flare the aircraft. The autothrottle mode transitioned to IDLE and both thrust levers were moving towards the idle position. At 08:37:12, 160 knots IAS, and 5 feet RA, five seconds before touchdown, the wind direction again started to change to a headwind.
The right main landing gear touched down at 08:37:17, approximately 1,100 meters from the runway 12L threshold at 162 knots IAS, followed three seconds later by the left main landing gear. The nose landing gear remained in the air.
At 08:37:19, the aircraft runway awareness advisory system (RAAS) aural message "LONG LANDING, LONG LANDING" was annunciated.
At 08:37:23, the aircraft became airborne in an attempt to go-around and was subjected to a headwind component until impact. At 08:37:27, the flap lever was moved to the 20 position. Two seconds later the landing gear lever was selected to the UP position. Subsequently, the landing gear unlocked and began to retract.
At 08:37:28, the air traffic control tower issued a clearance to continue straight ahead and climb to 4,000 feet. The clearance was read back correctly.
The aircraft reached a maximum height of approximately 85 feet RA at 134 knots IAS, with the landing gear in transit to the retracted position. The aircraft then began to sink back onto the runway. Both crewmembers recalled seeing the IAS decreasing and the copilot called out "Check speed." At 08:37:35, three seconds before impact with the runway, both thrust levers were moved from the idle position to full forward. The autothrottle transitioned from IDLE to THRUST mode. Approximately one second later, a ground proximity warning system (GPWS) aural warning of "DON’T SINK, DON’T SINK" was annunciated.
One second before impact, both engines started to respond to the thrust lever movement showing an increase in related parameters.
At 08:37:38, the aircraft aft fuselage impacted the runway abeam the November 7 intersection at 125 knots, with a nose-up pitch angle of 9.5 degrees, and at a rate of descent of 900 feet per minute. This was followed by the impact of the engines on the runway. The three landing gears were still in transit to the retracted position. As the aircraft slid along the runway, the No.2 engine-pylon assembly separated from the right hand (RH) wing. From a runway camera recording, an intense fuel fed fire was observed to start in the area of the damaged No.2 engine-pylon wing attachment area. The aircraft continued to slide along the runway on the lower fuselage, the outboard RH wing, and the No.1 engine. An incipient fire started on the underside of the No.1 engine.
The aircraft came to rest adjacent to the Mike 13 taxiway at a magnetic heading of approximately 240 degrees. After the aircraft came to rest, fire was emanating from the No. 2 engine, the damaged RH engine-pylon wing attachment area and from under the aircraft fuselage. Approximately one minute after, the captain transmitted a "MAYDAY" and informed air traffic control that the aircraft was being evacuated.
Together with the fire captain, the first vehicle of the airport rescue and firefighting service (ARFFS) arrived at the Accident site within one minute of the aircraft coming to rest and immediately started to apply foam. Additional firefighting vehicles arrived shortly after. Apart from the captain and the senior cabin crewmember, who both jumped from the L1 door onto the detached slide, crewmembers and passengers evacuated the aircraft using the escape slides, though not all exits could be used as some slides were blown up against the aircraft. The slide at the L1 door deflated after several passengers had evacuated.
Twenty-one passengers, one flight crewmember, and one cabin crewmember sustained minor injuries, and a second cabin crewmember sustained a serious injury.
Approximately nine minutes after the aircraft came to rest, a firefighter was fatally injured as a result of the explosion of the center fuel tank. Other 8 rescuers and firefighters received minor injuries.

A Malaysian military Sikorsky S-61A-4 Nuri helicopter, registration M23-33, crashed into a school Tuesday on Borneo island, injuring at least 23 people including students.
The helicopter made the emergency crash landing at a high school in Tawau in Sabah state about two hours into the routine training flight, the air force said. It said all 14 people on board the helicopter survived, and that it will investigate the cause of the crash.
The helicopter hit the roof of a school building before it crashed down onto part of the school canteen. He said the pilot was in critical condition and that all the military personnel have been hospitalized. Another eight students and a school worker also suffered light injuries and have been treated.

A Mi-8 helicopter, registered RA-22869 and owned by “Skol” company, crashed late Friday night 45 km NE of Old Urengoy. 19 people died from multiple injuries at the scene. Emergency workers were sent to the site of the crash and were able to save three from the wreckage, the regional branch of emergencies ministry said.  The three were admitted to a hospital in Novy Urengoy. Rescue teams found the two black boxes of the ill-fated helicopter. Окончательный отчет по результатам расследования авиационного происшествия

 

21 октября 2016 года, в 17:57 московского времени, в 45-ти км северо-восточнее п. Старый Уренгой Ямало-Ненецкого АО произошло авиационное происшествие с вертолетом Ми-8 RA-22869, принадлежащим ООО а/к «Скол» (Красноярское МТУ Росавиации).

Вертолет, выполнявший рейс СД 9375 с посадочной площадки Ванкор на посадочную площадку Старый Уренгой, не вышел на связь в установленное время и не прибыл в пункт назначения. Организованными поисками вертолет был найден разрушенным.

На борту находилось 22 человека. Три члена экипажа и 16 пассажиров погибли, три пассажира госпитализированы.

Причиной катастрофы вертолета явилась потеря экипажем пространственной ориентировки при попытке выполнения вынужденной посадки в ночных условиях при сплошной облачности и отсутствии световых ориентиров в месте посадки. Необходимость выполнения вынужденной посадки была обусловлена малым остатком топлива.

Вертолет Ми-8 (RA-22869), принадлежащ на ООО а/к „Скол”, изпълняващ полет СД 9375 от площадката за кацане Ванкор за Стария Уренгой превозвал служители на „Роснефт”. При опит за извънредно кацане се разбил. На борда му имало 22 души. Трите члена на екипажа и 16 пътници загинали на място, трима от пътниците са приети за лечение.

Разследването установи, че екипажът попаднал в лоши метеорологични условия и поради недостига на гориво се опитал да кацне принудително, но загубил пространствена ориентация и катастрофирал.

Парапланеристът Христо (37 г.) от В. Търново срива крилото си малко след излитане и пада според някои от 15-20 м, според други от 7-8 м върху асфалтов път. С тежки вътрешни натъртвания и увреждане на гръбначен прешлен е откаран спешно в Старозагорската болница за лечение. Предполага се, че инцидентът е резултат на грешка в управлението на крилото.

A Russian fighter jet crashed in the Mediterranean Sea shortly after launching from its aircraft carrier near the coast of Syria Sunday.
Three Russian MiG-29 fighter jets took off from their Soviet-era aircraft carrier, Admiral Kuznetsov, and flew in the direction of Syria. Once airborne, one of the Russian jets appeared to have mechanical difficulties and turned around in the direction of the aircraft carrier. The Russian jet splashed down in the water while attempting to land. A Russian rescue helicopter picked up a parachute and the pilot, who'd bailed out safely.

При проведении тренировочных полетов в результате технической неисправности во время захода на посадку за несколько километров до авианесущего крейсера "Адмирал Кузнецов" произошло авиационное происшествие с палубным истребителем Миг-29К. Летчик катапультировался. Поисково-спасательная служба сработала быстро. Вертолет Ка-27 взлетел через пять минут после сигнала о ЧП, без труда обнаружил на поверхности моря оранжевый надувной плот и поднял пилота на борт «вертушки». Еще через 15 минут он был уже на палубе крейсера.

Руски палубен изтребител МиГ-29К катастрофира в Средиземно море малко след излитане от самолетоносача си.

Група от три изтребителя са се отправяли към Сирия, но единият от тях, явно изпитвайки технически проблем, се е върнал обратно към самолетоносача „Адмирал Кузнецов”. Пилотът се е катапултирал едва на 10 м от водната повърхност. Аварийно-спасителната група е излетяла от самолетоносача с вертолет Ка-27 на петата минута. Лесно открили във водата оранжевия надувен плот и пилота. След още 15 минути се завърнали на борда на самолетоносача. Летецът е без наранявания и в добра кондиция.

A PZL-Mielec M28 Skytruck transport plane (P-4201), operated by the Indonesian Police crashed into the sea off Batam, Indonesia, killing all 13 occupants onboard.
The aircraft departed Pangkal Pinang Airport at 09.24 local time and was expected to arrive in Batam at 10.58. Its last known radar position was about 40 nautical miles southeast of Tanjung Pinang over the South China Sea.
Rescue officials said plane seats and suitcases along with body parts were recovered by fishermen off Riau Islands on Saturday afternoon.

4 декабря стало известно, что воздушное судно упало в Сингапурский пролив в районе острова Линга в провинции Архипелаг Риау.

На борту находились 13 человек - пять членов экипажа и восемь пассажиров; все они погибли.

Indonesian military plane crashes in Papua, 13 killedAn Indonesian military Hercules C-130 transport plane (A-1334) crashed Sunday in bad weather in the easternmost province of Papua, killing all 13 people on board.

The plane took off from Timika at 5:35 a.m. and crashed about four minutes before it was scheduled to land in Wamena.

The aircraft was carrying 12 tons of food supplies and cement from Timika to Wamena, a distance of about 200 kilometers (125 miles).

On board were three pilots and 10 other personnel.

The aircraft, purchased from Australia where it was first used in the 1980s, had more than 60 hours left until the next routine maintenance.

Bad weather and low clouds in Wamena, the capital of the mountainous district of Jayawijaya, were believed to be factors in the crash.

Транспортный самолет ВВС Индонезии С-130 „Геркулес” (регистрационный номер A-1334) разбился на востоке страны в провинции Папуа.
Все 13 человек, находившиеся на борту, погибли.
С-130, вылетевший из города Тимика около 05:35 по местному времени, врезался в гору неподалеку от места назначения - города Вамене. Самолет перевозил продовольствие. Причиной авиакатастрофы, по предварительным данным, могла стать плохая погода.
Тела всех погибших - трех пилотов и еще 10 человек - найдены и доставлены в Вамену.

Военно-транспоретн самолет Херкулес С-130 (A-1334) на индонезийските военно-въздушни сили с 13 души на борда и превозващ 12 тона храни и цимент от Тимика за Уамена се е разбил при подхода си за кацане на летището в Уамена. Няма оцелели. Предполага се, че причина за катастрофата са лошите метеорологични условия - ниска облачност в района на дестинацията.
