cargo plane crash 'leaves 15 dead' near Tehran

A cargo plane crash near the Iranian capital, Tehran, has killed 15
people, Iran's army says.
The Boeing 707 (EP-CPP) came off the runway and hit a residential area
while trying to land at Fath airport in Karaj, 40km (25 miles) west of
Tehran, in poor weather.
Only a flight engineer of the 16 people on board was found alive, with
no-one reportedly killed on the ground.
The plane's "black box" voice recorder has been found at the crash site.
The plane belonged to the air company Saha Air, and all those on board
were Iranian citizens.
Fath airport belongs to Iran's elite Revolutionary Guards Corps, and is
located in the central Iranian province of Alborz.
The cargo aircraft was transporting meat from the Kyrgyz capital,
Bishkek.
Its wreckage was found smouldering among houses in a residential area
between Fath and another airport, Payam International Airport. The
houses were empty at the time of the crash.

Бомбардировщик Ту-22М3 потерпел катастрофу при посадке
на аэродроме полка дальней авиации в Оленегорске.
Ту-22М3 - дальний сверхзвуковой ракетоносец-бомбардировщик с крылом
изменяемой стреловидности. Максимальная скорость бомбардировщика
достигает 2300 км/ч, дальность полета - 7 тыс. км.
Из четырех членов экипажа трое погибли.
Это командир экипажа м-р Алексей Гурьев, пом. командира экипажа м-р
Константин Мазунин, штурман-оператор кап. Виктор Грейф.
Единственный выживший член экипажа - штурман-инструктор подп.
Максим Рылков.

On February 8, 2019, at 12:16 EST, a General Dynamics Convair 340
(C-131B), N145GT, was destroyed during a ditching in the Atlantic Ocean
about 32 miles east of Miami-Opa Locka Executive Airport (OPF), Miami,
Florida. The captain Robert Hopkins, 68, of Coconut Creek, was fatally
injured, and the first officer Rolland Silva, 28, was seriously injured.
The airplane was registered to and operated by Conquest Air, Inc., as a
cargo flight. Visual meteorological conditions prevailed, and an
instrument flight rules (IFR) flight plan was filed. The flight departed
Lynden Pindling International Airport (MYNN), Nassau, Bahamas, at 11:13.
The accident occurred during a return trip to OPF. The cargo plane
started to lose altitude at about 4,600 feet and some 10 minutes later -
at 1,200 feet - it ditched into the ocean. It appeared the aircraft
broke up after it hit the water during an attempted water landing.
Investigators determined the aircraft was damaged beyond repair.
Atlas Air flight 5Y3591, a Boeing 767-300 (N1217A)
operated for Amazon Prime Air, was destroyed in a crash at Trinity Bay,
near Anahuac, Texas, USA.
All three on
board were killed.
The aircraft departed Miami International Airport, Florida at 11:33
hours local time (16:33 UTC) on a cargo flight to Houston-George Bush
Intercontinental Airport, Texas, USA. The cruising altitude of FL400 was
reached after 20 minutes. Descent towards Houston was commenced at 12:07
hours local time (18:07 UTC).
About 12:30 the pilots contacted the Houston terminal radar approach
control (TRACON) arrival controller and reported descending for runway
26L; the airplane was at 17,800 ft with a ground speed 320 knots.
At 12:34, the airplane was descending through 13,800 ft, and the
controller advised of an area of light to heavy precipitation along the
flight route and that they could expect vectors around the weather.
About 12:35, the flight was transferred to the Houston TRACON final
controller, and the pilot reported they had received the Houston
Automatic Terminal Information System weather broadcast. The controller
told the pilots to expect vectors to runway 26L and asked if they wanted
to go to the west or north of the weather.
Radar data indicated the airplane continued the descent through 12,000
ft with a ground speed of 290 knots, consistent with the arrival
procedure. The pilots responded that they wanted to go to the west of
the area of precipitation. The controller advised that to do so, they
would need to descend to 3,000 ft expeditiously.
About 12:37, the controller instructed the pilots to turn to a heading
of 270°. Radar data indicated the airplane turned, and the automatic
dependent surveillance-broadcast (ADS-B) data indicated a selected
heading of 270°. The airplane was descending through 8,500 ft at this
time.
About 12:38, the controller informed the pilots that they would be past
the area of weather in about 18 miles, that they could expect a turn to
the north for a base leg to the approach to runway 26L, and that weather
was clear west of the precipitation area. The pilots responded, "sounds
good" and "ok." At this time, radar and ADS-B returns indicated the
airplane levelled briefly at 6,200 ft and then began a slight climb to
6,300 ft.
Also, about this time, the FDR data indicated that some small vertical
accelerations consistent with the airplane entering turbulence. Shortly
after, when the airplane’s indicated airspeed was steady about 230
knots, the engines increased to maximum thrust, and the airplane pitch
increased to about 4° nose up. The airplane then pitched nose down over
the next 18 seconds to about 49° in response to nose-down elevator
deflection. The stall warning (stick shaker) did not activate.
FDR, radar, and ADS-B data indicated that the airplane entered a rapid
descent on a heading of 270°, reaching an airspeed of about 430 knots. A
security camera video captured the airplane in a steep, generally
wings-level attitude until impact with the swamp. FDR data indicated
that the airplane gradually pitched up to about 20 degrees nose down
during the descent.
Preliminary investigation results suggest that the captain may have
inadvertently activated the go around switch on the throttles as he was
moving his hand to select flaps.

The Indian Air Force
helicopter crashed into an open space in central Kashmir’s Budgam
district on Wednesday. The incident taking place around the time the
Pakistan Air Force conducted strikes at multiple places inside Indian
territory on the other side of the Pir Panchal mountains. The Pir
Panchal mountains separate the Valley from Jammu’s Pir Panchal region.
Eyewitness reported that a loud explosion was heard in the air before
the chopper crashed in a trail of smoke, indicating a possible
catastrophic external event contributed to the incident. Investigators have found that an Indian air
defence missile, was fired shortly before the crash of a
Mi17 V5 helicopter at Budgam, near Srinagar on February 27,
which resulted in the deaths of six air
force personnel and a
citizen on the ground. The Kashmiri youth 22-year-old
Kifayat Hussain Ganaie, a
brick kiln worker lost his life after he rushed towards the blazing
chopper to rescue the pilots after it had crashed.

Ninad Mandavgane (33), was
cremated on the bank of Godavari river. The investigators are examining the sequence
of events which preceded the crash. The incident had occurred on the
same day that India and Pakistan were locked in an air duel over
Nowshera in Jammu and Kashmir. The helicopter had crashed at Budgam as
it was approaching to land at Srinagar. The helicopter was downed by a
surface-to-air missile (SAM) of IAF, fired from the Srinagar air base
which assumed the helicopter to be a hostile platform. Two officers, the Chief Operating Officer
(COO) of the Srinagar Air Base and the Senior Air Traffic Control
Officer have been found blameworthy by a Court of Inquiry (CoI). The IAF
could possibly order the two officers to face a General Court Martial
which in turn is empowered to decide on the quantum of punishment,
including criminal charges, if any. The other three IAF personnel would face
disciplinary proceedings that could range from loss of seniority to
censure. The IAF Chief Air Chief Marshal BS Dhanoa,
had earlier removed the Air Officer Commanding (AOC) of the Srinagar air
base — in a move to show that procedural lapses will not go
“unpunished”. The Russian-made copter had crashed at 10.05
am, around the same time as Indian and Pakistani fighter jets were
engaging each other over Nowshera in Jammu and Kashmir. Wing Commander
Abhinandan Varthaman’s jet was downed around 10.30 am. The investigation indicated lapses that lead
up to the accident. The Air Traffic Control had called back the
helicopter as an air duel between Indian and Pakistani jets had
escalated. This is being seen as a lapse as the helicopter should have
been sent away to a safer zone instead of being called back to Srinagar.
Also, as the copter was returning, it should have been made to land at
the designated safe spot. On ground, the missile system and air defence
guns fire away at aircraft or helicopters which can’t be identified. The crucial identification or the Friend or
Foe (IFF) system on the Mi-17 helicopter was switched off, which was
against procedures.
The IFF is a system on board a helicopter or a plane
that automatically transmits a unique identification signal. That the IFF would remain mandatorily
switched on was a decision taken last year after a near mid-air accident
between a Sukhoi-30 fighter jet and a transport plane of the IAF. Though
the IAF issued instructions that the IFF system should be switched on,
the Srinagar air base had issued contradictory orders. The IAF
controllers on ground who were live monitoring the Pakistan air space
had not reported any intruding helicopter.

Aéreo Colombia Douglas DC-3 crashed in Colombia, 14 killed

The DC-3 aircraft with
registration HK2494, operated by LASER,
was prepared to make a non-scheduled passenger flight between Taraira
Airport (SKTR) and the aerodrome Villavicencio (SKVV) in Colombia. The
aircraft took off at 06:13 hours local time with a crew of three, eight
passengers and 500 kgs of cargo. Adverse weather conditions were
encountered en route and the crew decided divert to Miraflores (SKMF)
where it landed at 07:39. There two more passengers boarded the flight
for Villavicencio. The aircraft (10) passengers and the crew rescheduled
the flight to the Jorge Enrique Gonzales (ICAO: SKSJ) airfield in the
city of San Jose del Guaviare, to refuel and continue their flight to
(SKVV).
At 08:34 the aircraft took off from Miraflores and flew to San Jose del
Guaviare (SKSJ) to refuel, where it landed at 09:14. The aircraft was
fueled and the crew submitted a flight plan to Villavicencio with a
total flight time of 45 minutes an altitude of 8,500 feet. Prior to
departure one more passenger boarded the flight.
The aircraft took off at 09:55. At 10:15, when the aircraft was climbing
through 8,100 feet and approximately 59NM (109 km) from Villavicencio,
the flight reported engine problems and stated their intentions to
divert to a nearby airstrip. Villavicencio ATC suggested La Rinconada
Airstrip in the Municipality of San Martín, Meta, Colombia.
The aircraft lost altitude and last radio contact was at 10:32 when the
crew radioed that they had a runway in sight, with no further
information.
At 10:34 the aircraft hit the ground of a palm plantation with a high
angle of descent and low speed. Flaps and undercarriage were retracted
at the time of the accident. The aircraft slid across a gravel road and
burst into flames. All 14
occupants have perished, including the mayor of the
municipality of Taraira - Vaupes Doris Lizeth

плъзнал се по черен път и избухнал в пламъци. Всичките

A Kazakh military Mi-8 helicopter crashed on Wednesday
in Kazakhstan during exercises. The incident occurred when the
helicopter was flying from Aktau to Shymkent as part of a group of four
military helicopters. The copter has crashed due to unknown
circumstances, caught fire and was burned completely.
All 13 people on board the
helicopter were killed. A special commission has been
formed and sent to the crash site by Kazakhstan’s Defense Minister
Nurlan Yermekbayev.

По информации источника, по плану учений „Айбалта-2019” воздушное судно
в составе еще четырех вертолетов должно было лететь по маршруту
Актау-Бейнеу-Крайний-Шымкент. Спустя два часа после вылета с
промежуточного аэродрома Крайний вертолет разбился в районе поселка
Жалагаш Кызылординской области. На борту находились командир экипажа
Костыгов Р.П., правый летчик лейтенант Айтбаев А.Б., бортовой механик
Кудайбердиев Т.А. и 10 пассажиров. Перелет осуществлялся в темное время
и в сложных метеорологических условиях.
Командующий Военно-воздушными силами СВО Вооружённых сил РК Даурен
Косанов на пресс-конференции в СЦК объяснил, почему в разбившемся
вертолёте Ми-8 находились 13 человек и почему метеорологические условия
не могли вызвать проблемы у пилота.
"Для вертолётов армейской авиации сложные метеоусловия – это при высоте
ниже 200 метров. На период перелёта от Кызылорды до Шымкента высота
нижней границы была 390-600 метров. Это не является для них сложными
метеорологическими условиями", – сказал Даурен Косанов.
Погибший майор Роман Костыгов имел уровень подготовки для полётов в
пределах 100-метровой нижней границы при видимости 1 км. Перед ним
летели двое молодых капитанов, затем летел сам майор Костыгов, группу
замыкал подполковник Сагиев.
"Два опытных лётчика со вторым классом (Костыгов и Сагиев. – Авт.),
поэтому у них личный состав сидел. У двух капитанов по 3 человека
экипажа было, так как они лётчики 3-го класса. Эти два капитана смогли
пролететь. Я сомневаюсь, что лётчик 2-го класса, инструктор, заместитель
командира авиационной эскадрильи хуже летает, чем молодые капитаны. Они
(молодые пилоты. – Авт.) сели спокойно: один в Шымкенте, второго вернули
обратно в Кызылорду", – заявил Косанов.
Он пояснил, что рассматривают следующие версии крушения: отказ техники,
человеческий фактор и всё-таки метеорологические условия.
По роковому стечению обстоятельств в этом же самом районе 11 лет назад
разбился еще один Ми-8.
28 февраля 2008 года борт "Казавиаспаса" не
долетел до нужной точки. 27 марта и 28 февраля, с разницей в 11 лет,
стали печальными датами в истории страны.

маршрута Крайний един от тях се разбива в района на село Жалагаш.
Полетът бил по план на провежданото в страната учение „Айбалта-2019”
(Лунна брадва-2019) с участието на екипи от специалните подразделения
„Алтън жебе” (Златна стрела). Медии разкриват имената на екипажа -командир
м-р Костыгов Р.П, втори пилот лейтенант Айтбаев А.Б. и борден механик
м-р Кудайбердиев Т.А..
Два от четирите вертолета са се приземили в град Шимкент. Третият се е
върнал на местопроизшествието. Полетът се провел в сумрак и сложни
метеорологични условия. Води се разследване за изясняване на причините

Industries' L410UPV-E20, registration 9N-AMH,
owned and operated by Summit Air Pvt. Ltd. met with an
accident at Tenzing-Hillary Airport, Lukla when it veered right and
excursed the runway during take-off roll from runway 24. The aircraft
first collided with Manang Air's helicopter, AS350B3e,
registration 9N-ALC, with its rotor blade running on
idle power and then with Shree Airlines' helicopter, AS350B3e,
registration 9N-ALK just outside the inner perimeter fence of the
aerodrome into the helipad before coming to a stop. The PIC and Cabin
Crew of 9N-AMH survived the accident, whereas the Co-pilot and one
security personnel on ground were killed on the spot. One more security
personnel succumbed to injury later in hospital. 9N-AMH and 9N-ALC both
were substantially damaged by impact forces. There was no post-crash
fire. The Shree Airlines Eurocopter AS 350, 9N-ALK sustained minor
damage.
Lukla Airport is located on a mountain at an elevation of 9334 feet
(2845 m). The runway is 1729 ft long (527 m) with an 11.7% gradient. The Aircraft Accident Investigation Commission
determines that the probable cause of the accident was aircraft's
veering towards right during initial take-off roll. The veer was a
result of generation of asymmetric power due to abrupt shifting of right
power lever rearwards. The particular reason for abrupt shifting of the
power lever couldn‘t be identified.

flight 293, a Boeing 737-81Q, registration
N732MA, departed the end of runway 10 at Jacksonville
Naval Air Station (KNIP), Jacksonville, Florida, and came to rest in
shallow water in St. Johns River. There were no serious injuries
to the 142 passengers and crew onboard. The airplane was
substantially damaged. The airplane was operated by Miami Air
International as a non-scheduled passenger flight from Leeward Point
Field (MUGM), Guantanamo Bay, Cuba, to KNIP.
The pilot contacted the KNIP radar controller, and, at 21:39, the KNIP
radar controller indicated the winds were from 240° at 10 knots; he then
cleared the flight to land on runway 10. The pilot acknowledged the
landing clearance when the airplane was about 5 nautical miles from the
runway threshold. At 21:41, the KNIP radar controller instructed the
pilot to report the field in sight, to which the pilot immediately
responded he had the field in sight.
At 21:42 the aircraft touched down at an airspeed of 163 kts with a
tailwind of about 15 knots. The ground spoilers deployed three seconds
after touchdown. The aircraft was not able to use thrust reversers as
the right hand thrust reverser was inoperative. This was allowed per
Minimum Equipment List (MEL).
The airplane impacted a rock embankment at the end of the runway 10
overrun area before coming to rest in the St. Johns River about 1,200 ft
from the end of runway 10. The center of the airplane was about 100 ft
east of the river’s shoreline and about 75 ft south of the runway 28
landing light pier. The water where the airplane came to rest was 3-5 ft
deep, depending on the tidal conditions. The embankment had a distinct
area of disturbed rocks, consistent with the engines and main landing
gear impacting the embankment. The airplane was mostly intact, but both
main landing gear had separated from the airplane and were also located
in the river.

Seven Venezuelan military officers were
killed on Saturday when their helicopter crashed while
heading to a state where President Nicolás Maduro appeared alongside
troops, days after the opposition called in vain for a military
uprising. The Eurocopter AS 532 Cougar helicopter (reg.
59180) hurtled into a mountain outside Caracas in the
early hours of an overcast day in the capital. An investigation was
underway.
The armed forces in a statement said the chopper was heading to San
Carlos in Cojedes state. That's near a military academy where Maduro
appeared early Saturday to oversee training exercises following a week
of intrigue that saw a small group of security forces turn against him
in the failed attempt by opposition leader Juan Guaidó to overthrow the
government. On board the helicopter were two lieutenant colonels as well

The Beechcraft BE-300 King Air 350i (N534FF) was
destroyed after impacting a hangar during takeoff from runway 15 at
Dallas-Addison Airport (ADS/KADS), Dallas, Texas. A post-impact fire
ensued. The ten occupants onboard received fatal injuries. The
cross-country flight was originating at the time of the accident and was
en route to Albert Whitted Airport (KSPG), St. Petersburg, Florida. Inside the hangar a Dassault Falcon 900 jet (N908CA) and a helicopter
sustained some damage. The takeoff of the airplane was captured by radar and multiple security
cameras and was observed by several witnesses located in various
locations at the airport. One witness stated to the NTSB that as the
airplane went down the runway, it seemed more quiet than normal and
sounded like it did not have sufficient power to takeoff. After the
airplane lifted off, witnesses observed the airplane drift to the left,
and then roll to the left before colliding with the hangar. Several
security cameras captured the drift to the left immediately after
takeoff and then a roll to the left. The airplane ground speed decreased
from 114 knots when analysis started down to 85 knots shortly before the
airplane crashed into a hangar. One camera showed the airplane roll
completely inverted before it collided with the hangar. The empennage, right engine, and both propeller assemblies separated
from the airplane during the impact sequence and were located inside of
the hangar. Fragmented pieces of both wings were located on top of the
hangar, inside of the hangar, and immediately to the north of the
hangar. The main wreckage, which included the left engine and the
fuselage, was located outside of the hangar and it came to rest adjacent
to a brick wall. The main wreckage came to rest on its right side and
was destroyed by the impact forces and post impact fire. The cockpit voice recorder (CVR) recorded a crew comment regarding a
problem with the left engine about 8 seconds before the end of the
recording. Three automated "bank angle" aural alerts began about 3
seconds before the end of the recording. The victims included pilots
Howard Cassady (born on February 2, 1948 in Houston, Mississippi)
and Matthew Palmer (May 6, 1991);
Brian and
Ornella Ellard and their teenage children,
Alice Maritato and
Dylan Maritato;
Steve and
Gina Thelen; and John and
Mary Titus. All eight passengers were
members of Bent Tree Country Club in Dallas, according to an email the
club sent to members.
On 15 July 2019, the float-equipped de Havilland DHC-2 MK I
Beaver aircraft (registration C-FJKI, serial
number 992), operated by Air Saguenay
(1980) Inc. (Air Saguenay), was conducting a day trip under visual
flight rules from a fly-in fishing lodge on Crossroads Lake,
Newfoundland and Labrador, to Mistastin Lake, Newfoundland and Labrador.
There were 1 pilot, 2 guides and 4
fishermen on board the aircraft. The purpose of the flight was to travel to
Mistastin Lake for a day of fishing. Mistastin Lake is approximately 16
km by 11 km in area and over 200 feet deep in places, and is accessible
only by air. There was some fog and mist in the morning, and the flight
was delayed leaving the lodge, departing between 0900 and 0930.
When the aircraft did not return to the lodge in the evening, staff
attempted to communicate with the aircraft through a radio at the lodge,
and also through Air Saguenay's dispatch in Wabush, Quebec. When no
communication was established, the company implemented its emergency
procedure and first contacted the Joint Rescue Coordination Centre
(JRCC) in Trenton at 20:38. A search and rescue effort was initiated.
At around 05:00 on 16 July, the SAR aircraft spotted the occurrence
aircraft's fuselage floating in the water. The fuselage was inverted and
partially submerged. The engine, wings, and floats had detached from the
fuselage and were not visible. The bodies of 3 passengers were recovered
from the water, the temperature of which was 6 °C to 7 °C.
On 24 July, the body of a fourth passenger was recovered. None of the 4
passengers were found wearing personal flotation devices.
The search ended on 08 August 2019. The recovery team did not locate the
remaining occupants or the wreckage. The pilot and the 2 other
passengers are still missing.
The pilot, 61-year-old Gilles Morin,
had 20,000 hours of flying experience, according to Jean Tremblay, the
chief executive of his employer, regional airline Air Saguenay. Morin
had been flying in and out of Mistastin Lake, where the plane's wreckage
was found, for at least six years.
Pakistani army plane crashes
into homes, killing 19

A Pakistan Army Aviation Corps Beechcraft B300 King Air 350i
(766, serial № FL-766) was destroyed when it impacted
residential structures near Mora Kalu (10 km south of Nur Khan AFB) in
the residential area of Rawalpindi in Punjab. The plane was on a routine
training night flight.
According to a local rescue spokesman 19 dead bodies were brought to hospital that included 14
civilians and five crew members. Among the victims is a family whose
eight members, including four children, were killed. At least 15
people were also injured in the crash, some of them critically.
Rescue teams of 1122 and Pakistan Army immediately reached the site and
started the rescue operation.
The ISPR (Inter-Services Public Relations - the media wing of the
Pakistan Army) said that rescue teams had extinguished the fire caused
by the crash and moved the injured to various hospitals of Rawalpindi,
where paramedics said most of the victims were badly burnt.
President Arif Alvi and Prime Minister Imran Khan expressed their grief
over the loss of lives in the aircraft crash.
